Full program Descriptions

1915(i)

1915(i) Services are home and community-based services designed to help children and adults with mental health conditions, substance use disorders, intellectual and developmental disabilities (I/DD), or traumatic brain injuries live successfully in their homes and communities rather than in institutional settings. Services are individualized and recovery-focused and may include skill-building, employment supports, respite care, community transition assistance, and other supports that promote independence, community integration, and quality of life.

  • Assistance transitioning from institutional or congregate settings to independent housing

  • Community Transition Services that may assist with moving expenses, utility setup, deposits, and basic household items when transitioning to independent housing

What to Expect

Depending on eligibility and needs, services may include:

Community Living and Supports or Individual and Transitional Support to build skills such as budgeting, meal preparation, transportation use, household management, and social engagement
Supported Employment or Individual Placement and Support (IPS) to help find and maintain competitive employment
Respite Services to provide temporary relief for caregivers
Community Transition Services that may assist with moving expenses, utility setup, deposits, and basic household items when transitioning to independent housing

Substance Abuse Intensive Outpatient Program(SAIOP)

Our Substance Abuse Intensive Outpatient Program (SAIOP) is here to help people struggling with substance addiction. This program is offered three days a week for three hours per session. SAIOP provides you with the opportunity to participate in group therapy, which builds on education and the development of healthy life and coping skills. We also offer crisis planning and weekly random drug screenings. Our goal is to provide support in moving towards a life free from the hindering use of substances so that you can live a healthier and more fulfilling life.

Comprehensive Treatment

Substance Abuse Comprehensive Outpatient Treatment(SACOT)

Our SACOT program is a more intensive version of SAIOP that meets 5 days a week. We offer this program for people with complex medical or mental health needs with a long history of substance abuse. The program also provides you with the opportunity to participate in therapeutic and educational groups, individual sessions, and a higher level of involvement from medical professionals. Our goal is to help you achieve a lifetime commitment to healthy living without the use of drugs and alcohol.

Peer SUpport

Peer Support Services are recovery-focused behavioral health services provided by trained and certified peer support specialists who have lived experience with mental health challenges, substance use disorders, or both. These services help individuals build self-advocacy skills, increase community engagement, strengthen recovery supports, and achieve personal wellness goals. Peer Support Services promote hope, empowerment, and recovery through mentoring, coaching, skill development, resource navigation, and shared lived experience, helping individuals successfully manage their behavioral health needs and improve their quality of life within their communities.

What to Expect

Peer support workers are assigned to you. They are people who have been successful in the recovery process who help others experiencing similar situations. Through shared understanding, respect, and mutual empowerment, peer support workers help people become and stay engaged in the recovery process and reduce the likelihood of relapse. Peer support services can effectively extend the reach of treatment beyond the clinical setting into the everyday environment of those seeking a successful, sustained recovery process.

Child & Adolescent Day Treatment & Another Teen Zone

Our Day Treatment Program is a game-changer for school-age youth, blending academic excellence with personalized therapeutic care. We offer a school-based curriculum that's tailor-made for each child, with our dedicated staff advocating for their success in every aspect of life. With group and individual therapy sessions, we tackle mental and behavioral health head-on, empowering kids to overcome challenges and discover their true potential. Our program runs year-round, with exciting field trips, complimentary transportation, and meals to make the journey as smooth as possible.

But that's not all, introducing 'Another Teen Zone,' the next evolution in youth care! Designed specifically for teens aged 13-17, this spin-off of our Day Treatment program offers a safe, dynamic space where teenagers can navigate the complexities of growing up. Here, we focus on character development, educational achievement, and vocational goals, all while tackling the real issues teens face today. From hands-on activities that build life skills to group discussions that foster self-awareness, Another Teen Zone is where teens can thrive, grow, and prepare for the future.

Days and hours are Monday to Friday from 9:00 am to 3:00 pm during the summer and from 1:00 pm to 7:00 pm during the school year.

Community Support Team

Community Support Team (CST) services are intensive, community-based behavioral health services provided by a multidisciplinary team to adults with significant mental health and/or substance use needs. CST helps individuals develop the skills and supports necessary to live successfully and independently in their communities while reducing crises, hospitalizations, and institutional placements.

  • Reduced psychiatric hospitalizations and crisis episodes
  • Better medication adherence and understanding of treatment
  • Increased independence in daily living
  • Assistance accessing housing, employment, education, and community resources
  • Enhanced coordination among providers, family members, and support systems
  • Greater ability to achieve personal recovery goals

What to Expect

  • An assessment to identify strengths, needs, and goals
  • A person-centered treatment plan developed with the individual
  • Regular visits in the home or community rather than only in an office setting
  • Support with coping skills, problem-solving, and daily functioning
  • Crisis prevention and intervention when needed
  • Coordination with therapists, psychiatrists, primary care providers, and other services
  • Frequency of contact may vary based on need and can increase during periods of instability
  • Respite care: Breaks for caregivers, including overnight and weekend care 
  • Supported Employment: Helps you find and keep a job that's right for you 
  • Individual and Transitional Support: Helps you find housing, manage your finances and more 
  • Community Living and Support: Helps you learn to perform daily activities, like bathing and dressing 
  • Community Transition: Helps you move from an institutional setting back to your community
  • Individual Placement and Support Services: Job help for those with severe mental illness or substance use disorders
     

Outpatient TherAPY

Outpatient therapy provides structured behavioral health treatment for individuals experiencing mental health concerns, substance use challenges, emotional difficulties, life stressors, or relationship issues. Services are delivered utilizing evidence-based practices and may include individual therapy, group therapy, or a combination of both. Treatment is tailored to each person's needs, strengths, and recovery goals.

iNDIVIDUAL tHERAPY

  • Provides a confidential and supportive environment to discuss personal concerns 
  • Helps develop healthy coping skills and problem-solving strategies 
  • Improves emotional regulation, self-awareness, and resilience 
  • Supports management of symptoms related to depression, anxiety, trauma, substance use, and other behavioral health conditions

gROUP tHERAPY

  • Offers support and encouragement from others with similar experiences 
  • Reduces feelings of isolation and promotes connection 
  • Provides opportunities to practice communication and interpersonal skills
